Insurance coverage depends heavily upon each patient's individual health insurance.

In the event that a patient does not have insurance. A cash rate is available upon request.

 

Individual insurance plans:
 

We accept most major insurance plans including Group Health, Regence, Premera Blue Cross/Blue Shield, Aetna, First Choice, United, Cigna, Healthways, Lifewise and more. Please call our office to check on your individual plan.
* Please note that Medicare does not cover active release or other soft tissue modalities.

If treatment is for a Labor & Industries injury, make sure to bring:

• Your claim number

• The name of your claim adjuster

• Referral from your attending doctor

• Your health insurance card in case your claim is closed

 

If treatment is for a motor vehicle accident, make sure to bring:

• The name of your insurance company and claim number

• The name of your claim adjustor and phone number

• The name, phone number and address of your attorney (if applicable)
